First-Day Forms

The Forest Hills School District requests that the first-day packets for students be submitted electronically. To access the forms, please click below for your elementary students.  If you have multiple students in the district, please complete them for each student in the appropriate building.  Thank you for your cooperation.

Student Emergency Card - These will be distributed to students on the first day of school
Optional First Day Forms
Custody Agreements—Families must provide the District with custody agreements. The families must promptly furnish the district with any updates to these agreements and updated copies. The District can only adhere to the agreements provided by the families.
